We ask that you review the following patient information so that you will be aware of our procedures prior to your next appointment. If you have additional questions not covered in this brochure, please do not hesitate to contact us.
1. Appointments: We work by appointment only. Patients are required to arrive 15 minutes before an appointment time so that our personnel can verify personal and insurance information. However, emergencies and unexpected delays are at times unavoidable. Please call early in the morning for a same day appointment. If you cannot keep an appointment, please let us know as soon as possible, so that another patient can be scheduled.
2. Patient Information: Patient information is updated at every LFP office visit to prevent problems with insurance billing. You must inform the receptionist of any coverage change prior to your visit. The patient address and phone number is very important when trying to contact you concerning medical information.
3. Payment Policy: Payment is due the day services are rendered, this includes co-payments, co-insurance etc. Without proof of insurance coverage, payment in full is due at the time of service. If your deductible has been satisfied for the year, please provide us with a copy of an explanation of benefits from your insurance company stating your deductible has been met.
4. Refill Policy: If you must call our office for a refill of your prescription medication, you will be asked to leave a message with the following information:
Please allow 24 hours for your prescription to be called in. We will not contact you regarding your request for refill unless there is a problem or we have a question.
5. Emergencies: A physician will be available at all times (including nights, weekends, and holidays) for emergencies. An emergency is defined as any medical problem that the patient believes is an emergency. This service is strictly for emergencies or urgent needs, and not routine or minor problems that can be handled during office hours. If an emergency occurs after hours, please have a paper, pencil, and pharmacy phone number available. If your call involves medication, please state the name of the medication if possible. Prescription refills/requests will not be handled by the on-call physician nor will pain medications or controlled substances be called out.
6. Referrals: Your insurance company may require a referral for service rendered outside our office. You must get a referral prior to receiving those services, (i.e. specialist, lab, x-ray). If you have an appointment and need a referral authorization from your insurance company, allow us 72 hours to complete. If we are making the referral appointment and getting the insurance authorization, allow 7 working days to complete.
7. Forms Policy: Any form requiring a nurse/physician to complete (i.e. disability, family medical leave, insurance forms, etc.) will necessitate a charge. The first form filled out will be at no charge. Every form filled out thereafter will be charged at $10.00. Please allow 7 working days to complete. You may be asked to come in for an office visit to assist the physician filling out the form.
