Pre-Registration Phone Call
On this call, you'll need:
- Your Social Security number
- Insurance information
- Authorization number provided by your insurance company
Pre-Registration Interview & Assessment
Bring the following items with you:
- Medicines on your universal medication form, along with herbal products and over-the-counter medicines you take on a regular basis, as well as a record of dose and number of times taken
- Any other papers or instructions your doctor has given you.
Scheduling Your Medical Procedure
Scheduling for your procedure will be managed through your physician's office. Once your surgery has been successfully scheduled, your doctor will also provide you with an Outpatient and A.M. Admission booklet, along with any other information you may need.
Before your Scheduled Procedure
Insurance Issues
Upon approval for surgery, your insurance company will provide you with an authorization number. You will be required to provide that number at the time of preregistration, and again during your preadmission interview and assessment.
If you do not have the authorization number, your surgery may be delayed or rescheduled. Without authorization, any services performed may become your financial responsibility.
Pre-Registration Information Telephone Call
The Admissions department (LMC) or Outpatient Registrar (LMC Irmo or Lexington) will call to obtain information to register you for your surgery.
Pre-Registration Interview & Assessment
Our Interview and Assessment staff will schedule a convenient appointment with you to obtain pre-procedural health information and schedule any required pre-operative tests ordered by your physician, so that test results are back prior to your surgery.
- Patients are seen by appointment. Walk-ins will be worked in as our schedule permits.
- Minors (18 years and younger) must be accompanied by a parent or legal guardian.
- Plan on 1 to 1-1/2 hours for the interview process. This includes all lab work, X-rays and an EKG if ordered by your physician.
