Lexington Medical Center will fully comply with the Center for Medicare & Medicaid Services COVID-19 vaccination mandate for healthcare workers.

All non-LMC (Lexington Medical Center) staff, including, but not limited to; vendor/sales representatives, delivery personnel, contractors, independent medical staff, agency staff, and any other non-LMC representatives who have business or provide patient care, either under contract or other arrangements, at any Lexington Medical Center facility must be fully vaccinated or have met their organization’s COVID-19 vaccination exemption process, in accordance with CMS standards and federal law, before entering any Lexington Medical Center facility or practice location.

Lexington Medical Center’s Department of Public Safety will issue newly designed LMC ID badges to all non-LMC staff after they submit the required vaccination documentation or an acceptable exemption letter.

All non-LMC staff are required to have a new LMC ID badge to enter any LMC facility.

Steps to obtain a new non-LMC staff badge:

  1. LMC Directors and Managers will submit a list of authorized non-LMC staff, approving them to obtain the new LMC ID badge.
  2. Once non-LMC staff are fully vaccinated or have received an exemption from their employer, they must email the following information to vendorbadge@lexhealth.org:
    1. LEGIBLE copy of your proof of vaccination; or
    2. Signed letter from their employer’s President/CEO/designee attesting to their exemption status. The attestation letter must include the non-LMC staff member’s legal name as it appears on their valid identification. The attestation letter template is included in the policy.
  3. Non-LMC staff members will be notified within 1 to 2 business if there are errors or concerns regarding their submission. After two business days from successful e-mail submission, non-LMC staff members should go to Lexington Medical Center’s Department of Public Safety to have a photo taken and complete the process to obtain their new LMC ID badge.

No. You can visit the Department of LMC’s Department of Public Safety any time between the hours of 9:00 a.m. - 4:00 p.m. Monday through Friday (excluding holidays).

While there may be times of high volume, the process will typically take between 5-10 minutes. Please plan accordingly as you must show your valid form of identification (typically a driver’s license), have your photo taken and new ID badge printed. Do not wait until the last minute to pick up your badge.

You will need a valid form of identification (driver’s license, state ID card, etc.) and your current red-striped vendor badge, if you have one.

  • CDC COVID-19 vaccination card with your legal name clearly visible;
  • Documentation of vaccination from a health care provider or electronic health record; or
  • State immunization information system record.

No. You will need to pick up your new LMC identification badge in person.

No. Our physician practices and other departments are no longer permitting complimentary meals to reduce the potential spread of COVID-19. You should not bring food or other complimentary contributions to any of our facilities.
