Chronic Care Management

Managing your healthcare can be overwhelming, but it doesn’t have to be. If you live with two or more chronic conditions, such as diabetes, depression or high blood pressure, chronic care management can help you spend more time doing what you love.

Services Offered

  • Personalized assistance from a dedicated health care professional who will create a care plan with you including treatment goals, symptom management and medication management
  • Coordination between your pharmacy, service providers, specialists, testing centers, hospitals and others involved with your care
  • Phone check-ins between visits
  • Expert assistance to set and meet your health care goals
  • Covered by Medicare and supplemental insurance plans for eligible beneficiaries
  • Requires a monthly co-payment
  • Proactive care to avoid costly services in the future

Smoking Cessation Program

We offer the only cost-free smoking cessation program in the Midlands. Our sessions boast a 57% quit rate, compared to the national average of 17–23%.

Weekly Sessions (Every Tuesday, 6-8pm)

Sessions are held at various sites throughout the year to provide access for all the communities we serve.

Diabetes Education

Our ADA-accredited program is taught by a team of registered dietitians and specialists. We give patients skills and information to manage their diabetes and feel their best.

Class Topics

Classes are designed to help patients reach their diabetes wellness goals and are offered at a variety of locations in the Lexington Medical Center network. A provider referral is required.
