A New Hire Journey

Welcome Aboard!

We want to ensure that your journey with Lexington Medical Center starts smoothly. This page was designed to help you successfully navigate our new employee procedures.

Your journey as a new hire will be guided by the Human Resources department, Staff Development department and your department manager. With proper guidance, continued training and dedication, we know that your contributions to our team will continue in LMC's tradition of excellence.

First Things First

Please download and thoroughly review the New Journey Booklet, which contains detailed campus maps and other vital information regarding employee orientation and dress codes, along with further instructions on how to correctly fill out the required forms.

Orientation: All new employees are required to attend a General or Mini Hospital Orientation. If you haven't been advised of your orientation session schedule, please contact your HR recruiter for the date and time. View the Clinical Orientation schedule.

The following forms should be printed out and completed by hand, then returned to the HR Representative when you come in for your Post Offer Employee Health Screening.

  • Employee Personal Data Form: The Employee Personal Data Form contains information that will be used to start your electronic personnel record. To help us maintain the integrity of the data collected, please ensure that you complete each line of this form.
  • LMC Policies and Procedures Disclaimer Form: You will find two copies of this form in the downloadable file. Please ensure that you sign the disclaimer form designated as LMC Personnel Copy and keep the additional copy for your personal records.
  • Form I-9, Employment Eligibility Verification: Please complete Section 1 only – Employee Information and Verification. As this is a federal government form, special instructions apply; please refer to page 8 of the New Hire Booklet before completing the I-9 form. Once completed, please review, sign and date (within section one) only. When returning the completed form to LMC, you must also bring your government-issued identification.
  • Breach of Confidentiality, Privacy Notice & Privacy Notice Receipt Confirmation: Every LMC staff member is contractually obligated to protect patient and employee confidentiality.
  • Post-Offer Medical Questionnaire: Human Resources will schedule all new employees for a routine health screening, which will include a tuberculin skin test and a drug screen urinalysis. This form accompanies your checkup.

The following three forms should be printed out and completed by hand. Bring the completed forms with you to orientation.

  • NEW LMC Matching 401k Plan: LMC is proud to offer a NEW Defined Contribution Retirement Benefit option for New Hires in positions that are not required to join the SCRS or PORS.
